We are so happy to welcome current SMS families to complete the Enrollment Process for the 2012-2013 School Year.
Returning Preschool Students:
- Complete Enrollment Form and non-refundable $150 Enrollment Fee/child
- Submit newly completed State of Connecticut Early Childhood Health Assessment Record
Returning Kindergarten Students:
- Complete Enrollment Form and non-refundable $150 Enrollment Fee/child
- Pay Kindergarten Commitment Fee of $500, to be applied to Grade 1 tuition
- Arrange for completion of Teacher Observation Form
- Submit newly completed State of Connecticut Health Assessment Record
- Provide copy of Baptismal Certificate
Returning Grade 1-Grade 8 Student:
- Complete Enrollment Form and non-refundable $150 Enrollment Fee/child
