This checklist will guide you through the process of Applying to SMS Preschool Programs.
Our application process requires a number of steps for completion.
- Attend an Open House / Tour SMS
- Complete an Application Form and non-refundable $50 fee/per child.
If your child is accepted, you will need to:
- Complete Enrollment Form and non-refundable $150 Enrollment Fee/child
- Provide Copy of Birth Certificate
- Submit completed State of Connecticut Early Childhood Assessment Record
- Register for Smart Tuition service. Our account number is 11145.
New Students Applying to Saint Mary School
2012-2013 School Year
This checklist will guide you through the process of Applying to SMS Kindergarten – Grade 8.
Our application process requires a number of steps for completion.
- Attend an Open House / Tour SMS
- Complete an Application Form and non-refundable $50 fee/per child.
- Arrange for completion of Teacher Observation Form (Kindergarten only) or two Teacher Recommendation Forms (Grade 1-Grade 8)
- Complete an Authorization for Release of Records to obtain transcripts
- Arrange for student to visit SMS and have Screening
If your child is accepted, you will need to:
- Complete Enrollment Form and non-refundable $300 Enrollment Fee/child
- For Kindergarten, pay Kindergarten Commitment Fee of $1,000, to be applied to Grade 1 tuition
- Provide Copy of Birth Certificate and Baptismal Certificate
- Submit completed State of Connecticut Health Assessment Record
- Register for Smart Tuition service. Our account is 11145.
